Why does the answer say it depends on my plan?

Because for most commercial insurers it does. The insurer publishes the clinical criteria, and each employer decides separately whether the weight-management benefit is in the plan it buys. Two people holding the same card can get different answers, and no published document can tell you which one you have. The pharmacy-benefit number on the back of your card can.

Where do these answers come from?

Each one is read off the insurer's own published policy, formulary or plan document. Every answer carries the document number where the policy has one and the date a person here read it. Nothing is inferred from a press release or a secondary article.

My insurer is not on the list. What now?

Call the pharmacy-benefit number on the back of your card and ask two things: is the drug on the formulary, and what are the prior-authorization criteria. Those are the two facts every answer on this site is built from, and every insurer listed here has them read straight off its own published policy.
