Does Ambetter cover Wegovy and Zepbound?
No for weight loss in most states. Yes for a short list of other diagnoses. Wegovy is covered to lower cardiovascular risk, and for the liver disease MASH. Zepbound is covered for obstructive sleep apnea. Each one needs the plan to sign off first. California is the exception. Ambetter there is sold as Ambetter from Health Net, and it covers weight management and sleep apnea, both after a sign-off.

The Zepbound policy uses the same words. That same Wegovy policy does cover the drug to lower cardiovascular risk, and for MASH, a liver disease, once fibrosis is moderate to advanced. The Zepbound policy does cover Zepbound for moderate to severe sleep apnea. Each of those routes needs the plan to sign off first. California is the exception. Ambetter there is sold as Ambetter from Health Net. Its drug list says most plans do cover weight-loss medications after a sign-off, and the criteria set a marketplace threshold of BMI 40 or higher.

Does Ambetter cover Mounjaro?
Yes on some plans. Ambetter publishes a different drug list for each plan it sells, so two people with the same insurer can get different answers.
Yes for type 2 diabetes, in the states where Mounjaro is on the plan's drug list. The plan has to sign off, and you have to try other drugs first. Some state drug lists leave it off entirely. Ambetter marketplace plans run on Centene policy CP.PMN.183, which treats Mounjaro (tirzepatide) as a non-preferred agent. Approval needs a type 2 diabetes diagnosis, an A1C at or above 7 percent after at least 3 months on metformin, and a written record that the plan's preferred GLP-1 drugs failed first. Ambetter will not cover Mounjaro for weight loss. The FDA approved Mounjaro for type 2 diabetes only. The tirzepatide approved for weight loss is Zepbound, and Ambetter marketplace plans exclude it outside California.
