There are four savings levers for GLP-1s in 2026: the manufacturer savings card (commercial insurance only), the manufacturer direct-to-consumer cash program, the Medicare Bridge starting July 1, and telehealth first-month deals. Which one applies to you depends entirely on your insurance status.

Commercial insurance only. Manufacturer savings cards are not available to anyone covered by Medicare, Medicaid, VA, TRICARE or any federal or state government health program. Federal anti-kickback law prohibits it. Each section below restates this rule for the drug it covers.

Manufacturer savings cards

Every major GLP-1 has a savings card that drops the copay to as low as $25 a month for patients with qualifying commercial insurance. The card does not reduce what your insurer pays; it covers your out-of-pocket share up to an annual cap. You apply through the manufacturer's site or a specialty pharmacy, and most pharmacies apply it automatically once your card is on file.

You use either the manufacturer card or a GoodRx coupon on a given fill, never both. The card beats GoodRx in almost every insured case.

Cheapest cash, direct from the maker

Both Novo Nordisk and Eli Lilly run direct-to-patient cash programs that undercut retail pharmacy prices significantly. These are for patients without insurance coverage for the drug, or for patients whose insurance copay exceeds the direct price. You do not need insurance to use them.

Medicare: the GLP-1 Bridge starting July 1, 2026

The CMS GLP-1 Bridge Program begins July 1, 2026 and runs through December 31, 2027. It covers Zepbound, Wegovy, Foundayo at a flat $50 a month for eligible Medicare Part D members. Prior authorization is required.

This program is separate from the standard Part D obesity-drug exclusion. It does not require a cardiovascular disease or sleep apnea diagnosis. CMS has not yet published the full eligibility criteria as of June 15, 2026; check cms.gov or ask your Part D plan for details closer to the July 1 start date.

Before July 1, 2026, Medicare Part D covers Wegovy only for established cardiovascular disease (the SELECT trial indication) and Zepbound only for moderate-to-severe sleep apnea (SURMOUNT-OSA indication). Standard copays run $50 to $200 a month. The Part D obesity-drug exclusion still blocks coverage for obesity alone.

Manufacturer savings cards do not work with Medicare. If you are on Medicare and your drug cost is above $50 a month, ask your prescriber whether you qualify for the Bridge, or check the seniors guide for the full Medicare coverage picture.

GoodRx and pharmacy discounts

GoodRx discounts on brand-name GLP-1s are modest: typically 5 to 22 percent off the retail price. On a $1,349 Wegovy fill that means saving $67 to $297, which is less than the manufacturer savings card for most insured patients, and less than the NovoCare self-pay program for cash patients.

Two specific GoodRx behaviors to know:

  • Wegovy and Ozempic on GoodRx: GoodRx routes patients to the NovoCare self-pay program rather than a coupon code. You end up at the manufacturer program anyway.
  • Card vs coupon rule: you use a manufacturer savings card or a GoodRx coupon on one fill, not both. Stacking is not allowed.

GoodRx is most useful for generic liraglutide (Teva's Saxenda biosimilar, if available at your pharmacy), or for patients in edge cases where neither a manufacturer card nor a direct program applies. For branded GLP-1s, start with the manufacturer program before checking GoodRx.

Several telehealth programs that prescribe GLP-1s offer a lower first-month price to get you started. These are not coupons; they are introductory rates that reset after month one. The ongoing price is what you pay long-term, and that is the number that matters.

A first-month deal reduces the cost of trying a program, not the long-term cost. The all-in ongoing price at maintenance dose is what you will pay for month two and every month after. What is the cheapest way to pay for a GLP-1 with commercial insurance?

Use the manufacturer savings card for the drug your doctor prescribes. Wegovy, Ozempic, Zepbound, Mounjaro and Foundayo all have cards that drop your copay to as low as $25 a month. You cannot use a savings card together with GoodRx on the same fill -- use one or the other.

Can I use a GLP-1 savings card with Medicare or Medicaid?

No. Federal anti-kickback law prohibits manufacturer savings cards for patients covered by Medicare, Medicaid, VA, TRICARE or any other federal or state government health program. Using a card while on a government plan is illegal and can expose the patient and the pharmacy to liability.
