These rules come from the state preferred drug list (PDL), the list of drugs Michigan Medicaid has agreed to pay for. Prior authorization means your doctor asks the plan to approve the drug before the pharmacy fills it. Nearly every state covers the diabetes path that way.

What Michigan Medicaid paid for GLP-1s

Michigan Medicaid reported 149,092 GLP-1 prescriptions to CMS in 2025's last quarter and paid pharmacies $166 million.

What this means in practice

Michigan Medicaid covers Wegovy and Zepbound for weight loss, but it tightened the rules twice, on January 1, 2026 and again on July 1, 2026. Your doctor has to get approval before you start, the step known as prior authorization. You now need a starting BMI of 40 or higher. You also have to try five other weight-loss drugs on the state's list and have them fail. That is the slow part, so ask your doctor to start it early.

Managed-care organizations in Michigan

Michigan Medicaid pays private health plans, called managed-care organizations or MCOs, to run its coverage. The state contracts with 9, and these are the 5 largest. An MCO has to cover at least what the state list covers and can add rules of its own. Check your own plan's current rules before you send anything in.
