  • New-patient intro price: $199 a month. It covers the first two monthly fills of the Wegovy injection for new patients through December 31, 2026.
  • With insurance: $25 to $100 a month. A typical copay once a plan approves Wegovy for obesity. The hurdle is prior authorization.
  • Medicare from July 1, 2026: $50 a month. A flat price for qualifying Part D beneficiaries under the Medicare GLP-1 Bridge.
  • The cash gap is $1,200 a month. The distance between the $149 pill and the $1,349 list price, both of them branded Wegovy.

Compounded semaglutide: the $149 route the FDA has not approved

Compounded semaglutide is not Wegovy, and the FDA has not approved it. A licensed 503A pharmacy makes it against a prescription, and the FDA does not review it for safety, effectiveness or quality. Its formulation, including the salt form, can differ from the approved drug. Ask your prescriber which pharmacy makes it and which form of semaglutide it uses. It is the reason most cash-pay patients never pay the $1,349 list price.

The entry floor is $149 a month at Strut Health, billed month-to-month with no lock-in. That price covers the starter dose, and most programs charge more as you titrate up. If you want one price that holds across every dose, Shed runs $199 a month flat, the same rate at the starter and the maintenance dose. Found advertises $169 a month, but that rate is a year paid up front. Month to month Found is $289, and most other compounding programs land between $178 and $299. Noom Med pairs compounded semaglutide with its behavioral coaching program at $199 to $279 a month all-in, the lowest-priced compounding option that includes structured behavioral support. Supply has tightened through 2025 and 2026 as enforcement increased and some pharmacies paused programs, so check the live price before you sign up.

What Wegovy costs with insurance

When a commercial plan covers Wegovy for obesity, the copay usually runs $25 to $100 a month. That is the cheapest path of all, but it is gated by prior authorization. Most plans require a BMI of 30 or higher, or 27 with a weight-related condition, plus documentation; our prior authorization letter templates give your prescriber the wording plans look for. The oral pill at the starter dose is about $1,788 a year, and Shed charges $199 a month flat, or $2,388 a year. With insurance covering the drug, a year can cost as little as $300 to $1,200 in copays.

The gap between the cheapest brand route and full list price is $1,200 a month, or $14,400 a year, and both of those prices are for branded Wegovy. That gap is the whole reason to compare before you start.

Why Wegovy is so expensive in the US

The $1,349 list price is a US phenomenon. The same Wegovy costs a fraction of that in Europe. The US has no central drug-price negotiation, and pharmacy-benefit managers capture much of the rebate between list and net price. The list price is mostly a sticker.
