Prior-authorization criteria

These rules come from the state preferred drug list (PDL), the list of drugs Utah Medicaid has agreed to pay for. Prior authorization means your doctor asks the plan to approve the drug before the pharmacy fills it. Nearly every state covers the diabetes path that way.

What Utah Medicaid paid for GLP-1s

Utah Medicaid reported 5,350 GLP-1 prescriptions to CMS in 2025's last quarter and paid pharmacies $5.6 million.

Counts Wegovy, Zepbound, Ozempic, Mounjaro, Saxenda and Rybelsus, in fee-for-service and managed care, before rebates. CMS blanks its smallest rows, so the real figures are higher. Source: CMS State Drug Utilization Data.

What this means in practice

Utah Medicaid stopped covering GLP-1s for weight loss on June 30, 2026. A one-year pilot ran July 1, 2025 through June 30, 2026, funded by an appropriation rather than standing law, and it was not renewed. It also only ever applied to fee-for-service members, so anyone in a managed-care plan (Molina, SelectHealth, Healthy U, Health Choice) never had it. Utah's prior-authorization form was renamed 'GLP-1 Medications for Weight-related Comorbidities' on June 1, 2026 and the obesity approval criteria were removed, so there is no approval path left. Other uses continue: type 2 diabetes, heart-attack and stroke risk reduction (Wegovy injection or pill, BMI 27 or higher with a prior heart attack, stroke or symptomatic peripheral artery disease, and you must already be on prevention therapy), sleep apnea (Zepbound, BMI 30 or higher with an AHI of 15 or higher) and MASH, a fatty liver disease (Wegovy injection, fibrosis F2 to F3).

Managed-care organizations in Utah

Utah Medicaid pays private health plans, called managed-care organizations or MCOs, to run its coverage. An MCO has to cover at least what the state list covers and can add rules of its own. Check your own plan's current rules before you send anything in.

Cash-pay alternatives if Medicaid does not cover you

If you end up paying cash, the GLP-1 cost guide has what each cash route costs, from the compounded floor to the manufacturer-direct brand prices. Federal law bars manufacturer savings cards for anyone on Medicaid.

If a supply gap rather than cost is blocking your refill, the GLP-1 shortage tracker shows which formulations the FDA lists as in shortage now. And if you are 65 or older and also have Medicare, the senior GLP-1 hub lays out the Medicare coverage paths for dual-eligible patients.

Utah Medicaid GLP-1 FAQ

Does Utah Medicaid cover Wegovy for weight loss?
No. Utah Medicaid does not cover Wegovy for the weight-loss indication as of the 2026-07-01 preferred drug list review. If you have type 2 diabetes, the diabetes-indicated GLP-1s (Ozempic, Mounjaro) may still be covered with prior authorization.
Does Utah Medicaid cover Zepbound for weight loss?
No. Utah Medicaid does not cover Zepbound for the weight-loss indication as of the 2026-07-01 preferred drug list review. If you have type 2 diabetes, Mounjaro (tirzepatide, the same molecule as Zepbound) may be covered with prior authorization under the diabetes pathway.
Does Utah Medicaid cover Ozempic or Mounjaro for diabetes?
Yes. Utah Medicaid covers Ozempic and Mounjaro for type 2 diabetes with prior authorization, usually after a trial of metformin. The diabetes pathway does not carry the BMI threshold that applies to the weight-loss pathway.
What if Utah Medicaid does not cover my GLP-1?
If your GLP-1 is not covered, the routes left do not run through Medicaid. A clinical trial costs nothing and many pay you for your time. Nobody checks your income. Manufacturer savings cards are not a route here, because federal law bars them for anyone on Medicaid. The cheapest route left is cash-pay compounded semaglutide, a pharmacy-made version that is not FDA-approved and is not Wegovy or Ozempic. The prices checked October 12, 2026 are on the chart. If you were denied, the appeal letter library has denial-specific templates.

How we built this page

Compiled from each state Medicaid's published preferred drug list, PA criteria and PDL committee minutes. State Medicaid programs are not federally required to cover obesity drugs; the diabetes indication is covered nearly everywhere with PA. State-level data is point-in-time and shifts each PDL cycle. Always verify with your state Medicaid plan before relying on this page.

Checked August 24, 2026. We check each state Medicaid PDL and PA criteria quarterly. If you have recently had a different experience with Utah Medicaid, email hello@glpchart.com with the details and we will check it.

Why you can trust GLP Chart. Same scoring framework applied to every program. No paid placements. We never remove unfavorable information at an advertiser's request. Pricing is pulled from each program's public-facing page every Monday.
