Prior-authorization criteria
These rules come from the state preferred drug list (PDL), the list of drugs Maine Medicaid has agreed to pay for. Prior authorization means your doctor asks the plan to approve the drug before the pharmacy fills it. Nearly every state covers the diabetes path that way.

What MaineCare paid for GLP-1s
MaineCare reported 5,497 GLP-1 prescriptions to CMS in 2025's last quarter and paid pharmacies $5.2 million.

What this means in practice
MaineCare does not cover Wegovy for weight loss alone. It is covered for heart protection: BMI over 27 plus a prior heart attack, stroke or symptomatic peripheral artery disease (poor leg circulation), and the request must not be for weight loss only (PDL effective January 1, 2026). Zepbound is covered only for obstructive sleep apnea.
Managed-care organizations in Maine
Maine Medicaid operates primarily as fee-for-service. PA submissions go directly to the state Medicaid pharmacy benefit administrator rather than to a contracted managed-care organization (MCO), the kind of private health plan that runs Medicaid coverage in most states.
Cash-pay alternatives if Medicaid does not cover you
MaineCare covers Wegovy for a documented heart attack, stroke or peripheral artery disease, and covers Zepbound for sleep apnea. Neither drug has a weight-loss BMI route. Without one of those diagnoses, type 2 diabetes is the only route left.
