Prior-authorization criteria
These rules come from the state preferred drug list (PDL), the list of drugs New York Medicaid has agreed to pay for. Prior authorization means your doctor asks the plan to approve the drug before the pharmacy fills it. Nearly every state covers the diabetes path that way.

What New York Medicaid paid for GLP-1s
New York Medicaid reported 188,400 GLP-1 prescriptions to CMS in 2025's last quarter and paid pharmacies $215 million.

What this means in practice
New York Medicaid (NYRx) does not cover GLP-1s for weight loss alone. State law does not let Medicaid pay for a drug whose only job is weight loss. Two narrow exceptions sit on the drug list revised July 16, 2026. Wegovy if you have diagnosed heart disease. Zepbound if you have diagnosed sleep apnea. Both need a BMI of 40, and New York allows only two rounds of treatment in your lifetime. Obesity on its own does not qualify at any BMI. If you were denied, an appeal is the realistic next move.
Managed-care organizations in New York
New York Medicaid pays private health plans, called managed-care organizations or MCOs, to run its coverage. The state contracts with 16, and these are the 6 largest. An MCO has to cover at least what the state list covers and can add rules of its own. Check your own plan's current rules before you send anything in.

Cash-pay alternatives if Medicaid does not cover you
New York will not pay for a GLP-1 for weight alone, so the realistic routes are a documented heart-disease or sleep-apnea diagnosis, the type 2 diabetes path, or paying cash. Compounded semaglutide starts around $149 a month.
