Prior-authorization criteria

These rules come from the state preferred drug list (PDL), the list of drugs Vermont Medicaid has agreed to pay for. Prior authorization means your doctor asks the plan to approve the drug before the pharmacy fills it. Nearly every state covers the diabetes path that way.

What Vermont Medicaid paid for GLP-1s

Vermont Medicaid reported 2,912 GLP-1 prescriptions to CMS in 2025's last quarter and paid pharmacies $3.2 million.

Counts Wegovy, Zepbound, Ozempic, Mounjaro, Saxenda and Rybelsus, in fee-for-service and managed care, before rebates. CMS blanks its smallest rows, so the real figures are higher. Source: CMS State Drug Utilization Data.

What this means in practice

Vermont Medicaid excludes drugs used for weight loss. Wegovy is covered only for heart protection: a BMI over 27 plus a prior heart attack, stroke or symptomatic peripheral artery disease (poor leg circulation), and not for weight loss alone. Zepbound is covered only for moderate to severe obstructive sleep apnea: a BMI over 30, an AHI over 15 and current CPAP use, again not for weight loss alone.

Managed-care organizations in Vermont

Vermont Medicaid operates primarily as fee-for-service. PA submissions go directly to the state Medicaid pharmacy benefit administrator rather than to a contracted managed-care organization (MCO), the kind of private health plan that runs Medicaid coverage in most states.

Cash-pay alternatives if Medicaid does not cover you

If BMI threshold not met, the diabetes path applies for T2D.

If you end up paying cash, the GLP-1 cost guide has what each cash route costs, from the compounded floor to the manufacturer-direct brand prices. Federal law bars manufacturer savings cards for anyone on Medicaid.

If a supply gap rather than cost is blocking your refill, the GLP-1 shortage tracker shows which formulations the FDA lists as in shortage now. And if you are 65 or older and also have Medicare, the senior GLP-1 hub lays out the Medicare coverage paths for dual-eligible patients.

Vermont Medicaid GLP-1 FAQ

Does Vermont Medicaid cover Wegovy for weight loss?
No. Vermont Medicaid does not cover Wegovy for the weight-loss indication as of the 2026-08 preferred drug list review. If you have type 2 diabetes, the diabetes-indicated GLP-1s (Ozempic, Mounjaro) may still be covered with prior authorization.
Does Vermont Medicaid cover Zepbound for weight loss?
It depends. Coverage of Zepbound for weight loss under Vermont Medicaid is limited and can vary by managed-care plan. Prior authorization is required. Check your specific plan's drug list. Last state review: 2026-08.
Does Vermont Medicaid cover Ozempic or Mounjaro for diabetes?
Yes. Vermont Medicaid covers Ozempic and Mounjaro for type 2 diabetes with prior authorization, usually after a trial of metformin. The diabetes pathway does not carry the BMI threshold that applies to the weight-loss pathway.
What if Vermont Medicaid does not cover my GLP-1?
If your GLP-1 is not covered, the routes left do not run through Medicaid. A clinical trial costs nothing and many pay you for your time. Nobody checks your income. Manufacturer savings cards are not a route here, because federal law bars them for anyone on Medicaid. The cheapest route left is cash-pay compounded semaglutide, a pharmacy-made version that is not FDA-approved and is not Wegovy or Ozempic. The prices checked October 12, 2026 are on the chart. If BMI threshold not met, the diabetes path applies for T2D. If you were denied, the appeal letter library has denial-specific templates.

How we built this page

Compiled from each state Medicaid's published preferred drug list, PA criteria and PDL committee minutes. State Medicaid programs are not federally required to cover obesity drugs; the diabetes indication is covered nearly everywhere with PA. State-level data is point-in-time and shifts each PDL cycle. Always verify with your state Medicaid plan before relying on this page.

Checked August 24, 2026. We check each state Medicaid PDL and PA criteria quarterly. If you have recently had a different experience with Vermont Medicaid, email hello@glpchart.com with the details and we will check it.

Why you can trust GLP Chart. Same scoring framework applied to every program. No paid placements. We never remove unfavorable information at an advertiser's request. Pricing is pulled from each program's public-facing page every Monday.
