The Monday price check: September 21 2026

Sam's Club sells members the Zepbound pen for $399 a month at 5mg. Move up to 7.5mg and the same pen is $499. From 10mg up it's $699. That's $300 more a month for the same drug.

Every one of these drugs starts you on a low dose and steps you up over about four months. Novo tested a 7.2mg semaglutide shot against the standard 2.4mg dose. Over 72 weeks the higher dose took off 18.7% of body weight, the standard dose 15.6%.

The FDA added the 7.2mg step to the Wegovy label in June. Novo sells it as Wegovy HD for $399 a month. The standard pen is $349.

Novo's pill runs the same way. The Wegovy tablet is $149 a month at the 1.5mg starter dose, $199 at 4mg and $299 at the dose you stay on. Lilly's Zepbound vials run $299, $399 and $449 by dose. The ad prices the starter dose.

Two programs on the chart charge one price at every dose. Shed is $199 a month for compounded semaglutide. Henry Meds is $297 for the same drug, at every dose it publishes.

The new pill landed with the same ladder

Lilly's Foundayo pill is $149 a month on the Walgreens, GoodRx, Hims, Ro, Sesame and WeightWatchers pages. Ro added it last week. The $149 is Lilly's offer, so it's the same figure everywhere. The membership on top runs from $39 at GoodRx to $149 at Hims and Ro. GoodRx printed the rest of the ladder in its footnote. $149 buys the 0.8mg and 2.5mg doses, 5.5mg is $199, and every dose above that is $299. Walgreens and GoodRx both print the end date, December 31.

Also this week

One in nine American adults is on a GLP-1 for weight loss right now. Gallup has it at 11% this year, up from 3% in 2024. Brand-name drugs are 68% of that and compounded is 19%.

McDonald's is building a menu around them. Its chief executive told Harvard Business Review that people on a GLP-1 eat 700 to 800 fewer calories a day, and that adults are ordering Happy Meals. The chain is testing smaller portions and more protein.

Nothing on the chart moved this week. $149 has been the cheapest price on it for 14 checks running.
