Zepbound dosing: 2.5 mg to a 15 mg ceiling in 2.5 mg steps
Zepbound starts at 2.5 mg once weekly for 4 weeks, moves to 5 mg, then rises 2.5 mg at a time and never faster than every 4 weeks. The label names 5 mg, 10 mg and 15 mg as maintenance dosages for weight reduction, 10 mg or 15 mg for obstructive sleep apnea, and 15 mg as the maximum for every indication. The 2.5 mg starting dose is for initiation and is not approved as a maintenance dosage.

How long you stay on a Zepbound dose has no upper limit
This is where the tirzepatide labels part from Wegovy's. Wegovy prints a week-by-week table. Zepbound writes a rule: at least 4 weeks on the current dose before the next 2.5 mg step. There is no upper limit on how long you stay. The label carries no pause sentence. It says instead: "If patients do not tolerate a maintenance dosage, consider a lower maintenance dosage."
SURMOUNT-1, the clinical trial behind the weight-reduction indication, put 2,539 adults on tirzepatide or placebo for 72 weeks with a 20-week escalation at the front, and reported the gastrointestinal events as mostly mild to moderate and "occurring primarily during dose escalation." Discontinuation from adverse events ran 4.3 percent at 5 mg, 7.1 percent at 10 mg and 6.2 percent at 15 mg, against 2.6 percent on placebo. The 10 mg arm dropped out more than the 15 mg arm did, so the curve is not a clean line.
