Missed a GLP-1 dose? The window is 4 days on tirzepatide and 5 on Ozempic

Every GLP-1 label answers the missed-dose question and the answers do not match. Zepbound and Mounjaro give you 4 days (96 hours) to take it. Ozempic gives you 5 days. Wegovy states its rule backwards from the others: take it if the next scheduled dose is more than 2 days away, skip it if it is less. The daily products say skip it and take the next one tomorrow.

A longer gap is a different question

Missing several weeks is common, whether a pharmacy ran dry or a prior authorization lapsed. Wegovy and Saxenda are the only labels that tell the prescriber what to do about it.

  • Wegovy. If 2 or more consecutive doses are missed, the label says to reinitiate dosage escalation at a lower dosage, to reduce the risk of gastrointestinal adverse reactions. It does not name which lower dosage, so that part is the prescriber's judgment.
  • Saxenda. The strictest of the six. More than three days since the last dose and the label says to restart at 0.6 mg daily and climb the whole table again.
  • Zepbound, Mounjaro, Ozempic and Rybelsus. Silent. We searched the full structured product labels for every form of reinitiate, re-escalate, restart and interruption and found nothing. A restart protocol for tirzepatide comes from clinical practice, because the label does not contain one.

How late can I take a missed Zepbound or Mounjaro dose?

Within 4 days, which both labels write as 96 hours. Past 4 days, skip it and take the next dose on the regularly scheduled day.

How late can I take a missed Ozempic dose?

Within 5 days. Past 5 days, skip it and take the next dose on the regular day.

Why is the Wegovy rule written differently?

It is keyed to the next dose rather than the missed one. Take the missed injection if the next scheduled dose is more than 2 days away, and skip it if the next dose is less than 2 days away.

What if I have missed several weeks?

Two of the six labels answer that and four do not. Wegovy says to reinitiate dosage escalation at a lower dosage after 2 or more consecutive missed doses, without naming which. Saxenda says more than three days off and you restart at 0.6 mg. Zepbound, Mounjaro, Ozempic and Rybelsus say nothing about it, so a restart plan for those comes from your prescriber and not from a label.

Can I change which day I inject?

Zepbound and Mounjaro allow it if at least 3 days, 72 hours, separate the two doses. Ozempic allows it if at least 2 days, more than 48 hours, separate them. Wegovy's label says the time of day and the injection site can both change without a dosage modification.
