GLP-1 cash price index: $333 a month in September 2026

The median US GLP-1 telehealth program charged $333 a month in September 2026, according to GLP Chart's GLP-1 Cash Price Index, published October 4, 2026. Across the 18 programs priced in both August and September, the median was down $5. The cheapest verified compounded semaglutide route was $199 and the cheapest verified branded route $299.

Buying rather than citing? Strut Health is the cheapest cash route on the chart, from $149 a month with auto-refill. That price buys compounded semaglutide, a pharmacy-made version that is not FDA-approved and is not Wegovy, Ozempic, Zepbound or Mounjaro.

  • Corrected. Costco / Success by Sesame, Found, Lemonaid Health, Ro Body, Sesame Care and Shed carry the rate GLP Chart publishes today back into August.
  • As published. The September 2026 release put the August median at $348 under method 1.0, and that page stands unchanged.

What a pharmacy pays for the same month

The CMS National Average Drug Acquisition Cost survey records what US retail pharmacies pay their wholesaler. It is an acquisition-cost benchmark. It is not pharmacy margin: the distance to a cash price also covers dispensing, the clinician visit, shipping and the seller's own costs, and a manufacturer selling direct does not buy through a wholesaler at all. NADAC dashboards have been published free by 46brooklyn Research since 2019. What is added here is the join to verified consumer cash prices.

Method, version 2.0

  • Universe. Every program in the GLP Chart index that is active, prices in cash rather than an insurance copay, and publishes an all-in monthly price.
  • Compounded semaglutide route. Programs publishing one flat all-in monthly price covering compounded semaglutide at every dose. Entry and starting-dose prices are excluded.
  • Branded route. Programs whose primary supply is branded medication and whose maintenance pricing is verified rather than estimated.
  • The monthly value. Each week we take the median, or the lowest, price across every program priced that week, and the month's value is the median of those weekly figures.
  • Month on month. The change from the prior month counts only programs priced in both months, with any price we later corrected carried back into the prior month, so a program joining, leaving or being corrected never reads as a price move.
  • Product changes. A program that changes which drug or product its price covers counts as leaving and joining again, and the switch is named with its date and both prices.
  • What changed in version 2.0. Version 2.0 replaces version 1.0's fixed panel. Version 1.0 took the median over the programs priced in every weekly check since May 2026 and set it against the prior month's figure, so a program leaving, a program returning or a price we corrected could move the headline.
  • Prepaid rates. An observation equal to a program's published 12-month prepaid monthly rate is dropped, and a program whose only published rate is a block of months bought up front is left out.
  • Excluded. Insurance copays, introductory and starter-dose rates, prepaid plan rates, and programs that publish no ongoing price.
  • Classification. Programs are classified by the pricing model they publish today; the weekly prices are the historical record, read with every recorded correction applied.
