The figure comes from the National Average Drug Acquisition Cost file, which the Centers for Medicare & Medicaid Services publishes every week. It is what pharmacies pay to buy the drug. It is not what they charge, and it is not their margin. The file we used is dated September 2, 2026, with prices effective August 19.
Zepbound costs a pharmacy $482 to $1,052 a month. The spread is the pen: the multi-dose pen works out to about $482 a month at the 2.5mg to 7.5mg doses, against about $1,052 for single-dose pens. The lowest verified Zepbound cash price on our chart in October was $449 a month.
Ozempic's list price is $1,027.51 a month on Novo Nordisk's own pricing page, read October 29, 2026. List price is what the maker charges before any discount or insurance.
Cash prices now run far below these numbers because both makers sell direct at a discount. The Wegovy pill runs $149 to $299 a month by dose from the maker, and Zepbound vials $299 to $699. Compounded semaglutide starts at $59 a month for the drug on our chart.
Every price and its condition is on the chart. If your plan just dropped or denied a GLP-1, start with the employer plans guide.
