Does Premera Blue Cross cover Mounjaro?

Yes, with prior approval. Premera pays for Mounjaro once your prescriber gets the plan to sign off.

Premera covers Mounjaro for type 2 diabetes with prior authorization. Under pharmacy policy 5.01.569 (effective July 1, 2026), Mounjaro is medically necessary for adults and children aged 10 or older who have a type 2 diabetes diagnosis and have had an adequate trial of metformin or a contraindication to it, and it cannot be used with another GLP-1 or GIP/GLP-1 drug. Premera does not cover Mounjaro for weight loss and calls that use investigational. Zepbound is the tirzepatide product for weight loss, governed under a separate policy (5.01.621).

Premera Blue Cross covers Ozempic for type 2 diabetes with prior authorization. The plan requires the patient to be 18 or older, have a type 2 diabetes diagnosis, and have an adequate trial of metformin or a contraindication to it, and it applies a quantity limit at the FDA-approved maximum dose. Premera does not cover Ozempic for weight loss and calls that use investigational. Wegovy is the weight-loss form of semaglutide, and Premera reviews it under a separate weight-management policy that applies only when the employer group elects weight-loss drug coverage.

Premera Blue Cross covers Rybelsus for type 2 diabetes with prior authorization. Under pharmacy policy 5.01.569 (effective July 1, 2026), Rybelsus is a first-line GLP-1 that is medically necessary for adults aged 18 or older who have a type 2 diabetes diagnosis and have had an adequate trial of metformin or a contraindication to it, and it cannot be used with another GLP-1 or GIP/GLP-1 drug. Rybelsus is FDA-approved for type 2 diabetes only, so Premera does not cover it for weight loss. The weight-loss semaglutide is Wegovy, which Premera reviews under a separate policy (5.01.621) that applies only when the employer group elects weight-management drug coverage.
