Does Premera Blue Cross cover Wegovy?
Yes on some plans. Each employer picks whether Premera Blue Cross covers Wegovy, so two people with the same card can get different answers.
Premera covers Wegovy once the plan signs off. But it only covers it where your employer bought the weight-loss drug benefit.

Does Premera Blue Cross cover Mounjaro?
Yes, with prior approval. Premera pays for Mounjaro once your prescriber gets the plan to sign off.
Premera covers Mounjaro for type 2 diabetes once the plan signs off. Pharmacy policy 5.01.569, in force from July 1, 2026, sets the rules. It calls Mounjaro medically necessary for adults, and for children aged 10 or older, who have a type 2 diabetes diagnosis. They also need an adequate trial of metformin, or a contraindication to it. It cannot be taken with another GLP-1 or GIP/GLP-1 drug. Premera does not cover Mounjaro for weight loss, and it calls that use investigational. Zepbound is the tirzepatide for weight loss, and it runs under a separate policy, 5.01.621.

Premera Blue Cross covers Ozempic for type 2 diabetes once the plan signs off. You have to be 18 or older, with a type 2 diabetes diagnosis. You also need an adequate trial of metformin, or a contraindication to it. A quantity limit holds you to the FDA-approved maximum daily dose. Premera does not cover Ozempic for weight loss, and it calls that use investigational. The weight-loss form of semaglutide is Wegovy. Premera reviews Wegovy under a separate weight-management policy, and that one only applies where the employer group bought weight-loss drug coverage.

Premera Blue Cross covers Rybelsus for type 2 diabetes once the plan signs off. Pharmacy policy 5.01.569, in force from July 1, 2026, sets the rules. It treats Rybelsus as a first-line GLP-1. It calls the drug medically necessary for adults aged 18 or older who have a type 2 diabetes diagnosis. They also need an adequate trial of metformin, or a contraindication to it. It cannot be taken with another GLP-1 or GIP/GLP-1 drug. The FDA approved Rybelsus for type 2 diabetes only, so Premera does not cover it for weight loss. Wegovy is the semaglutide for weight loss. Premera reviews Wegovy under a separate policy, 5.01.621, and that one only applies where the employer group bought weight-management drug coverage.
