Questions to ask your doctor about a GLP-1
Print this page and bring it to your visit. Write the answers in the lines. Clinics and support groups may print and hand it out for free.
Is a GLP-1 right for me?
- Do I qualify for a GLP-1 for weight, for diabetes or both?
- Which one fits me best: semaglutide (Wegovy, Ozempic) or tirzepatide (Zepbound, Mounjaro)? Shot or pill?
- Does anything in my health history or my other medicines rule one out?
Side effects and safety
- Which side effects are common at the start, and how long do they usually last?
- Which signs mean I should call you or get care right away?
- Should I change how I eat, drink or exercise while I take it?
Dose and the plan
- What dose do I start on, and when does it go up?
- How will we know it is working, and when do we check in?
- What happens if I stop, or miss a dose?
Cost and insurance
- Will you send a prior approval request if my plan needs one?
- If my plan says no, will you help with an appeal letter?
- Can you write the prescription so I can fill it with cash if that is cheaper?
- Is a compounded version an option for me, and what is different about it?
