Compare what GLP-1 programs charge this week
The index takes the lowest price each week across the programs that publish one flat monthly drug price for compounded semaglutide at every dose, then the median of those weeks. In October that floor was Collective's $59 monthly drug price, first read on October 12. In September it was $198.
The brand-name floor did not move. The lowest verified brand-name maintenance price on our chart was $299 a month in every month from May to October 2026, so the gap between brand-name and compounded is now $240 a month.
The median all-in cash price across the programs we track was $299 in October, down from $333 in September. Most of that fall comes from new, lower-priced programs entering the index. On the programs priced in both months, the median did not change.
Brand-name cash prices now sit well below what a pharmacy pays to stock the drug. The federal acquisition-cost file puts a month of Wegovy at about $1,302 to $1,307 for a retail pharmacy, and a month of Zepbound at $482 to $1,052 depending on the pen.
The release, the CSV and the method are on the GLP-1 price index. This week's price for every program is on the chart, and what changed is in the biggest price drops of October.
