How to get the Medicare GLP-1 Bridge: six steps from prescription to the first $50 fill
You do not sign up for the Medicare GLP-1 Bridge. Your prescriber sends the prescription to your pharmacy with a note that routes it to the Bridge, the pharmacy bills Medicare, and your prescriber completes a prior authorization. CMS decides within 72 hours of getting the form.

The six steps
- Your prescriber writes the prescription for Wegovy, Foundayo or the Zepbound KwikPen, for weight loss.
- They add the routing note. An obesity diagnosis code from the E66 family, and SEND TO BRIDGE FOR WEIGHT MANAGEMENT in the note field. Without it the pharmacy may bill your Part D plan, which can reject the claim or start its own review.
- Your pharmacy bills the Bridge. It may ask for your Medicare number, or look it up from the last four digits of your Social Security number.
- The Bridge asks for approval. It turns down the first claim on purpose and tells the pharmacy to request a prior authorization. The pharmacy sends the form to your prescriber, usually within 24 to 72 hours.
- Your prescriber sends the form back, online through CoverMyMeds or by fax. The decision reaches them within 72 hours and comes to you by mail.
- You pay $50 at the pharmacy for a one-month supply of 28 or 30 days.

After the first fill
- Refills need no new approval while you stay on the same drug, even when the dose goes up. The approval runs to December 31, 2027.
- Switching drugs, say Wegovy to Foundayo, needs a new prior authorization.
- There are no 60 or 90-day fills, and lost or stolen doses are not replaced early.
- No coupon or discount card can lower the $50.
- A refill is refused if you later fill a GLP-1 through your Part D plan, or move to a plan type the Bridge does not take.
There is no appeal. If the form was turned down, your prescriber can send it again with corrected or new information.
What to take to your prescriber
Your BMI when you started, any diagnosis of type 2 diabetes, sleep apnea or MASH, and the name of the drug. Ask for the routing note by name. Run the checker first so you walk in knowing which band you are in.
