Type 2 diabetes, sleep apnea or MASH: why the Bridge sends you to Part D
If you have type 2 diabetes, moderate to severe sleep apnea or MASH, you cannot use the $50 Medicare GLP-1 Bridge, whatever your BMI. CMS sends all three to your Part D plan, which may already cover a GLP-1 for the condition at your plan's own copay.

Why CMS draws the line here
Part D is barred by law from paying for a drug used for weight loss, and the Bridge exists for that gap. The five situations above are ones Part D can cover, so CMS keeps them with your plan.
A past heart attack or stroke does not shut you out. It is one of the BMI 27 conditions. What decides the route is why the drug is prescribed: for weight, the Bridge; to cut the risk of another heart attack or stroke, your Part D plan, even if weight is a reason too.
CMS can check. The Bridge may confirm a diagnosis such as type 2 diabetes against Medicare's own records, and it looks for any GLP-1 your Part D plan paid for in 2026.
How the Part D route differs
- Your plan sets the copay, the tier and any prior authorization, so the price is your plan's, not a flat $50.
- A Part D fill counts toward your yearly out-of-pocket limit, which is $2,100 in 2026. A Bridge copay does not.
- Extra Help lowers a Part D copay if you qualify. It does not lower the Bridge copay.
Ask your plan two things: is the drug your prescriber wants on its formulary for your diagnosis, and does it need prior authorization.

Can I use the Medicare GLP-1 Bridge if I have type 2 diabetes?
No. CMS excludes type 2 diabetes even when your BMI fits a band. Your Part D plan may already cover a GLP-1 for diabetes.
I have sleep apnea and a BMI over 35. Which route is mine?
Your Part D plan. Moderate to severe sleep apnea keeps you out of the Bridge whatever your BMI.
What if my Part D plan turns the drug down?
You can appeal through the plan, or pay cash. The Bridge stays closed to you while the diagnosis stands.
