Can you use an HSA or FSA for Wegovy, Zepbound or compounded GLP-1s?
Usually yes, with a prescription. IRS Publication 502 counts prescribed medicines and drugs as medical expenses, and counts weight-loss costs when they treat a disease a physician diagnosed, such as obesity. Your HSA or FSA plan administrator decides each claim.

What the IRS says
“You can include in medical expenses amounts you pay for prescribed medicines and drugs. A prescribed drug is one that requires a prescription by a doctor for its use by an individual.” (IRS Publication 502, Medicines)
“You can include in medical expenses amounts you pay to lose weight if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease).” (IRS Publication 502, Weight-Loss Program)
Publication 969 says an FSA pays the qualified medical expenses its plan names, which generally follow these medical expense rules. Under Publication 502, gym, health club and spa dues do not count, diet food does not count, and supplements count only when a practitioner recommends them for a diagnosed condition.
What usually counts, and what to check
| Cost | HSA or FSA? |
|---|---|
| Wegovy, Zepbound, Ozempic, Mounjaro or the pills, on a prescription | Generally yes (prescribed drug) |
| Compounded semaglutide or tirzepatide, on a prescription | Not named by the IRS; it is a prescribed drug (our reading). Ask your administrator |
| A yearly or monthly program membership | Ask your administrator; the IRS does not name it |
| Gym dues and diet food | No |
Administrators commonly ask for the prescription and an itemized receipt; what yours needs is in its claim rules.

HSA and FSA GLP-1 FAQ
Is Wegovy HSA eligible?
With a prescription, generally yes. IRS Publication 502 says: "You can include in medical expenses amounts you pay for prescribed medicines and drugs. A prescribed drug is one that requires a prescription by a doctor for its use by an individual." Your HSA administrator decides each claim.
Is Zepbound FSA eligible?
The same rule applies to an FSA. Publication 969 says an FSA pays the qualified medical expenses the plan names, which generally follow the medical expense rules in Publication 502. Check your FSA plan's list.
Can I use an HSA for compounded semaglutide or tirzepatide?
The IRS text does not name compounded drugs. They are dispensed on a prescription, which is the test Publication 502 sets for drugs (our reading, not IRS wording). Ask your administrator. Compounded drugs are not FDA-approved.
Does the telehealth membership count?
Publication 502 counts weight-loss program fees when the program treats a disease a physician diagnosed, such as obesity. It does not name telehealth memberships. Your administrator decides whether one counts.
Do I need a letter of medical necessity?
The IRS text read here does not require one. Because Publication 502 ties weight-loss costs to a diagnosed disease, an administrator may ask for the diagnosis; a prescriber can write it down.
What happens to FSA money at year end?
An FSA is use-it-or-lose-it unless your plan offers a grace period of up to 2 1/2 months or a carryover. For 2025 plan years the IRS carryover cap was $660 (Publication 969, 2025 edition); your plan may set less, and its deadline is in your plan documents.
