FDA gave Mounjaro a heart attack and stroke indication in type 2 diabetes on a trial where it tied Trulicity

Mounjaro was measured against Trulicity, not a placebo, in 13,299 people over a median of 210 weeks.

Eli Lilly announced the approval on August 28. The prescribing information revised in August 2026 carries a second indication: reducing the risk of major adverse cardiovascular events in adults with type 2 diabetes who are at high risk of them. The label defines those events as cardiovascular death, non-fatal heart attack or non-fatal stroke. The original indication, blood sugar control in type 2 diabetes, is unchanged.

The trial is SURPASS-CVOT. Everyone enrolled had type 2 diabetes that was not well controlled and established cardiovascular disease. The comparator, dulaglutide, sells as Trulicity, an older Lilly drug that already carried its own cardiovascular indication, so the trial asked which of two Lilly drugs does more. The average participant was 64 years old, with an HbA1c of 8.4 percent and a BMI of 33. Follow-up ran a median of 210.1 weeks, about four years.

Of 6,647 people taking Mounjaro, 803 had a cardiovascular death, a non-fatal heart attack or a non-fatal stroke. Of 6,647 taking Trulicity, 863 did. That is 12.1 percent against 13.0 percent, a hazard ratio of 0.92 with a 95.3 percent confidence interval running from 0.83 to 1.01. The trial met its non-inferiority test at p equals 0.007. Superiority was not established, because that interval reaches past 1.00 and cannot rule out no difference between the two drugs.

A hazard ratio of 0.92 reads as an 8 percent lower risk, and the 8 percent is measured against dulaglutide, a drug that already lowers cardiovascular events, so the comparison starts from a treated baseline. The label declines to call that 8 percent established. The approval rests on non-inferiority to a GLP-1 that already has a proven heart benefit. In counted events the gap is 60, across 6,647 people in each arm over about four years.

For what you pay, this changes less than it sounds. The indication covers type 2 diabetes, so it adds no weight-loss claim and does nothing to Zepbound, which is the same molecule approved for weight loss. A patient already covered for Mounjaro on a diabetes diagnosis gains one more line for a prior authorization appeal. A cash price moves when a manufacturer or a pharmacy changes it, and neither changed here. What Mounjaro costs sets the retail price beside every cash tirzepatide route.
