Recent change: Policy 5.01.569 was last revised June 22, 2026 and took effect July 1, 2026. It sets a minimum age of 10 for Mounjaro.

Does Premera cover Mounjaro?
Premera covers Mounjaro for type 2 diabetes with prior authorization. Under pharmacy policy 5.01.569 (effective July 1, 2026), Mounjaro is medically necessary for adults and children aged 10 or older who have a type 2 diabetes diagnosis and have had an adequate trial of metformin or a contraindication to it, and it cannot be used with another GLP-1 or GIP/GLP-1 drug. Premera does not cover Mounjaro for weight loss and calls that use investigational. Zepbound is the tirzepatide product for weight loss, governed under a separate policy (5.01.621). This reflects the published policy verified August 2026.
