- The policy sets no A1C threshold and no mandatory prior-drug failure requirement for approval.
- Approvals last up to 12 months.
- Not approved for a sole diagnosis of obesity. The tirzepatide product for weight loss is Zepbound, covered under a separate anti-obesity policy.
