What the prior authorization needs:
- Prior authorization required.
- Medical records confirming a type 2 diabetes diagnosis, shown by one of: A1C 6.5% or higher, fasting plasma glucose 126 mg/dL or higher, 2-hour plasma glucose 200 mg/dL or higher during an oral glucose tolerance test, or random plasma glucose 200 mg/dL or higher with classic symptoms of hyperglycemia.
- For patients requiring ongoing treatment diagnosed more than 2 years ago, medical records confirming the type 2 diabetes diagnosis.
- No step therapy in the current program. The metformin trial requirement was removed in the April 2024 update.
- Authorization is issued for 12 months.

Recent change: Effective 7/1/2026 under program 2026 P 1332-8 (P&T approval 4/2026). The April 2024 revision removed step therapy from this program, so a metformin trial is no longer required. Some third-party summaries still list a metformin step, which the current policy does not.

Does UnitedHealthcare cover Rybelsus?
UnitedHealthcare and OptumRx cover Rybelsus for type 2 diabetes under commercial plans, but it requires prior authorization. Approval needs medical records confirming a type 2 diabetes diagnosis, shown by an A1C of 6.5% or higher, a fasting plasma glucose of 126 mg/dL or higher, or another qualifying glucose value; patients diagnosed more than two years ago can qualify with records confirming the diagnosis. Rybelsus is not covered for weight loss, because it is FDA-approved only for type 2 diabetes and weight-loss drugs are typically excluded from the benefit. Wegovy is the weight-loss version of semaglutide. This reflects the published policy verified August 2026.
