Recent change: The prior authorization requirement for GLP-1 drugs on Florida Blue commercial plans took effect April 1, 2025. Members already on GLP-1 therapy at that date now need a prior authorization on file.

Does Florida Blue cover Ozempic?
Florida Blue covers Ozempic for type 2 diabetes on its commercial plans, but only with prior authorization. The provider has to document a type 2 diabetes diagnosis and show the patient tried and failed metformin, a metformin combination drug, or insulin first. Ozempic is not covered for weight loss. The weight-loss semaglutide is Wegovy, which Florida Blue excludes from coverage. This reflects the published policy verified August 2026.
